Dankzij het grote vakmanschap van zijn advocaat Kleinfeld, komt de harde crimineel Carlito Brigante na een aantal jaren in de gevangenis te hebben gezeten vrij. Deze keer is Carlito vastbesloten op het rechte pad te blijven. Hij wil zich terugtrekken op de Bahama's met zijn grote liefde Gail en de kost verdienen met het verhuren van auto's. De corrupte Kleinfeld wil hem wel helpen. Hij heeft een klusje waarmee Carlito een leuk startkapitaal kan verdienen. Hij noemt het slechts een kleine gunst, uit loyaliteit voor bewezen diensten. Maar als Carlito iets geleerd heeft uit het verleden, dan is het wel dat loyaliteit dodelijker is dan een kogel.
